New York Pain Specialist PLLC (“the Practice,” “we,” “us,” or “our”) uses EMRbyMD to provide transactional SMS messages related to your appointments and scheduling.

This messaging program is not a marketing program. Messages may include:

• Appointment confirmations
• Appointment reminders
• Scheduling-related transactional messages

2. Consent to Receive SMS Messages

By affirmatively authorizing SMS messaging, you consent to receive transactional text messages from New York Pain Specialist PLLC through EMRbyMD.

Message frequency may vary depending on your appointments and scheduling activity.

Message and data rates may apply.

Consent to receive SMS messages is not a condition of receiving medical care.

3. Opting Out
